Earning Loyalty Program points

•	Each month your patients use KOVALTRY^®, they will earn 1 Loyalty Program point

Introductory Enrollment Bonus—12 points:
When patients enroll during the introductory period,
they can get a year's worth of Loyalty points

The Loyalty Program is open to current KOVALTRY^® patients, as well as new patients to KOVALTRY^®. The Loyalty Program enables patients to accumulate points that can be redeemed for therapy:

•	When patients experience challenges getting insurance coverage for KOVALTRY^®

•	When patients are between jobs and experience a gap in insurance coverage

•	When patients are uninsured or underinsured

Redeeming Loyalty Program points

•	Points are your patients' protection against a lapse in insurance coverage. They may redeem 3 points for a 1-month supply of a Bayer product or multiple points in increments of 3, up to 12 points at a time^†

Bayer Access Solutions also offers a variety of programs that can help you assist your patients, including:

•	Free Trial Program^‡: Talk to your patients about requesting a free trial of KOVALTRY^® with Vial Adapter

•	$0 Co-pay Program^§: Eligible patients can receive up to $12,000 in assistance per year

•	Live Helpline Support: You or your patients can call for answers to any insurance coverage questions

Patients who have government insurance (Medicaid, Medicare, Tricare, VA/DOD) are not eligible for the Loyalty Program. The program does not guarantee that patients will be successful in obtaining coverage for product. Support medication provided through Bayer's assistance programs is at no cost to patients and is not contingent on future KOVALTRY^® prescriptions. Reselling or billing any third party for free product provided by Bayer's patient assistance programs is prohibited by law. Bayer reserves the right to determine eligibility, monitor participation, determine equitable distribution of product, and modify or discontinue the program at any time.

Patients may redeem multiple points as long as they do not exceed the maximum levels for that product.

The KOVALTRY^® Free Trial Program is available to newly diagnosed patients and patients who are currently using other therapy. Patients currently using KOVALTRY^® are not eligible for the Free Trial Program. Participation in the KOVALTRY^® Free Trial Program is limited to 1 time only. The medication provided through this program is at no cost to patients and is not an obligation to purchase or use KOVALTRY^® in the future. Reselling or billing any third party for the free product is prohibited by law.

People with private, commercial health insurance may receive KOVALTRY^® co-pay or co-insurance assistance based on eligibility requirements. The program is on a first-come, first-served basis. Financial support is available for up to 12 months. Eligible patients can re-enroll for additional 12-month courses. The program is not for patients receiving prescription coverage for product under any federal-, state-, or government-funded insurance programs, or where prohibited by law. All people who meet these criteria are encouraged to apply. Bayer reserves the right to discontinue the program at any time.
